Our Responsibilities

We are required by law to maintain the privacy of your PHI, provide you with this Notice, and comply with its terms.

We comply with applicable federal and Texas state privacy laws, including those that provide additional protections for certain types of health information.

We will notify you following a breach of unsecured PHI, as required by law.

Telehealth and Electronic Communication

We may provide healthcare services via telehealth and communicate with you electronically. Reasonable safeguards are used to protect your privacy; however, no system is completely secure.

Changes to This Notice

We reserve the right to change this Notice and make the revised Notice effective for all PHI we maintain. Updated versions will be available upon request and in our office.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with The Lamp Family Health or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
